So , what explains the rapid wear-away on the engine side of the clutch plate ?
Was it a 'cheap' clutch plate ?
Or 'something ' restricting the movement of the plates ?
If you don't know , why won't it happen again ?

For that to happen it must have been slipping right from the time of fitting.
Did you ever feel it not providing any engine braking on the over-run , or poor acceleration ?

The dirty coolant is not a good sign , I hope it's not too late to flush and refill with the proper coolant. The proper stuff is not cheap but a lot less than another engine . Make sure all the linkages and moving parts in the accelerator are moving as they should . That tiny ballrace in the quadran...

Do triple check that the slave cylinder IS pushing the clutch push rod , etc, properly before doing the big job.
